Terms 

1. Video classes are rented assuming 4 students will view the video, and receive "Certificates of Completion" after passing a short test.
2. The Company Requesting the Video Class will be responsible for showing the video in DVD format in the appropriate environment, with a facilitator who has reviewed the video before the class.
3. Rental is for 6 weeks after the DVD is sent to the requesting company.
4. Rental includes DVD, and training materials for 4 students.
5. Additional students can be supported for a cost of $125 per student.
6. Additional persons can view the DVD but will not be issued "Certificates of Completion", even if the student passes the test.
7. Payment (P.O., check, credit card number) is due prior to shipping the DVD to the Company Requesting the Video Class.

• DVD Rental $500 for 4 Students, $125 for each additional student
